Thank you for being part of Upbeat Arts.

We really value you and your experience at our programs.

This survey takes around 5-10 minutes to complete. You are welcome to have someone help you, such as a support worker, family member, or staff member.

Your responses can be anonymous. Staff at your program will not see individual responses. They will only receive a summary of feedback from all members, unless you request follow up support. Please share what feels right for you.

In the survey below, when we use the word “program”, we mean the Upbeat Arts program you attend. This may be one or more of our choirs and/or writing groups. If you attend more than one program, please answer the questions based on your experience across all programs you attend.
